Authorization to Obtain Consumer Credit Report

hereby authorize Iconic, to verify my past and present employment earnings records, bank accounts, stockholdings, and any other asset balances that are needed to process my consumer dispute/reconciliation.

I/We further authorize Iconic, to order a consumer credit and background check report and verify other credit and background check information, including but not limited to past and present mortgages, landlord references.

Iconic may also verify all my personal credit information and background information. It is understood that a copy of this form will also serve as authorization. This authorization expires 120 days from the date indicated.
